Who is eligible?

You'll be eligible for an aged care assessment if you are:

  • Over the age of 65
  • An Aboriginal or Torres Strait Islander person over the age of 50
  • A younger person with a disability, dementia or other care needs not met through other specialist services.

If you don't meet these requirements, call My Aged Care on 1800 200 422 to discuss your circumstances.

How to apply

If you're seeking government-funded home care services, you'll need to apply for an in-person assessment with My Aged Care. Or, contact us to organise private services.

Check your eligibility

To check your eligibility for an aged care assessment, call My Aged Care on 1800 200 422.

Apply for an assessment

If you're eligible, call 1800 200 422 or complete My Aged Care's online application (allow 15 to 20 minutes).

Prepare for your assessment

Visit the My Aged Care website for a full list of things you should prepare before your assessment.

Have your assessment

Your aged care assessment will take place in your home. Consider having a support person such as a family member or your carer with you.

Get your assessment outcome

Your assessment letter will show your approved aged care services and provide your individual support plan.

What happened to Home Care Packages?

On 1 November 2025, the Australian Government replaced Home Care Packages and Short-Term Restorative Care with the Support at Home program.

Support at Home was released on 1 November and is designed to deliver services maintain your independence, safety and wellbeing at home and in the community.

What changed with Support at Home?

On 1 November 2025, the Support at Home program introduced:

  • Single assessment for all home care services
  • 8 levels of funding so care matches your needs
  • Fairer pricing and payment rules for all older Australians
  • Budgets every 3 months, with automatic care coordination

If you were receiving a Home Care Package, or on the National Priority System, or assessed as eligible for a Package on 12 September 2024, you're a Grandfathered Home Care Package recipient.

This means you were automatically transitioned to the new Support at Home program on 1 November 2025 without losing your current funding or benefits.

A ‘no worse off’ principle applies to HCP grandfathering, meaning that you will not be worse off under the Support at Home program, and you won’t make higher participant contributions than you did for your Home Care Package. 

Note: Grandfathering arrangements only apply to your participant contributions and primary supplements.

 

Transitioned HCP recipients were either receiving a Home Care Package or on the National Priority System prior to 1 November 2025, then transitioned to Support at Home on 1 November 2025. 

If you were a Short-Term Restorative Care client before 1 November 2025 and your current Service Agreement ends after 31 October 2025, we will continue to provide services to you in line with your current support plan until your current service period ends.

If you receive your home support through the Commonwealth Home Support Program (CHSP), you will move to Support at Home no earlier than 1 July 2027, unless you are reassessed for a higher level of services.
