Uniting Community Living Supports (CLS): Northern Sydney

Are you 16 or over, and living with severe mental health challenges that impact your daily life?

We can work with you to develop a plan that supports you to live and participate in your community, the way you want to.

Program information

Build your skills, confidence and community connections

We work in partnership with you to set goals that will support change and improve wellbeing across different areas of your life.

One of our support workers, a clinician, your family, and any other people that are important to you, can help to develop your support plan.

It’s based on your own unique needs, preferences and goals, and can include getting support for:

  • Shopping, cooking, taking public transport and other daily activities
  • Life skills, including looking after your finances
  • Reminders about appointments, medication and other treatments
  • Meeting people in your community and social activities
  • Accessing education, training opportunities or finding a job
  • Moving back home after a stay in hospital or prison
  • Accessing drug and alcohol services, the National Insurance Disability Scheme (NDIS) or other support services.

This program does not provide social or community housing. Learn more about how to get assistance with housing.

As much support as you need

At times, you might feel that a couple of hours a week is enough, and other weeks you might find 5 hours a day is useful to you.

Am I eligible?

You may be able to access this free program if you:

  • Are 16 or over
  • Have been diagnosed with a mental health condition which is impacting your day-to-day life
  • Have a local mental health case manager or are in contact with the community justice system
  • Are able to meet regularly with your support provider in the Northern Sydney area

  • Putting your social housing at risk
  • Coming out of prison or are in contact with the community justice system (for example, you might be on parole or serving a community-based detention order)
  • Living in a boarding house
